Let me explain how a typical process will go...

#1: The customer and I will discuss said project, usually through email and I will send them a cover art form which asks for pertinent information such as genre, character coloring, type of feel generated in the cover, etc etc.

 #2: I will work up a mockup of said cover and send it to customer, usually at 300X400 dimensions. I will go up to 3 mockups, so please plan accordingly. If the work is deemed extensive, the cost will be higher but I will not do any further work until the customer is made aware of the change in cover art cost.

#3: Once customer and I have come to an agreement, I will send the cover art as a jpeg file in three formats, 200x300, 800x1200 and 1600x2400.

#4: I will then send an invoice through Paypal for 50 USD to be paid within three days of cover receipt. I do accept payments through bank transfers (in fact, I kind of prefer this), but not mailed checks as I live overseas.

I reserve the right to refuse any requests I do not feel confidence in fulfilling to the customer's satisfaction.
